Affordable Refund IVF Program

Using current criteria, our center has achieved an 84% success rate with our refund IVF program. These phenomenal success rates are a direct result of the individual attention that we give our patients and the clinical and laboratory excellence of our staff. The following sections will address the specifics of how the refund IVF program works.

What this program is: This is a way for couples to address the high cost of IVF services with the assurance that they will have the financial possibility of other options if IVF does not work for them.

What this program is NOT: It is not a guarantee of pregnancy or delivery of a healthy baby. Medical outcomes cannot be guaranteed.

Requirements to Participate in the Program:

• Wife must be age 37 or younger

• Wife must not have had more than 1 prior IVF "start"

• BMI 29 or less (this is a standardized way of measuring weight – charts can be found on the web)

• Normal ovarian reserve

• No medical condition which would make a pregnancy (or twin pregnancy) unwise

• No genetic abnormalities of either partner

• Able to finish program in 9 months

• Normal uterine cavity and lining

How the Program Works

The total contract fee for standard refund is $17,100 for women less than 35 years of age and $19,000 for women ages 35 to 37. A portion of this fee (30%) is a non-refundable deductible. If the couple is unsuccessful in the program (i.e., the couple does not have a baby reaching 1 month of age from the date of delivery), the contract fee minus the deductible (i.e., the "refundable fee") will be returned to the patient. It is important to note that the couple may not proceed into another IVF stimulation cycle until all frozen embryos from the prior stimulation cycle have been utilized. We may give exception to this rule on an individual basis. Please note that any Frozen Embryo Transfer cycles performed prior to completion of the contract are INCLUDED in your contract price. You WILL NOT pay extra for your frozen embryo transfers during your contract period. This represents a significant savings to our patients. Also note that the contract is not completed until all embryos from all 3 retrievals, including frozen embryos, are transferred or a delivery has occurred.

Please note: if a pregnancy results but the child does not reach 1 month of age (30 days) from the date of delivery, the couple will be permitted to complete the terms of the contract. In the event that the couple does not become pregnant again after completing all remaining stimulation cycles and utilizing all frozen embryos, they will be eligible to receive the refundable portion of the contract fee.

What is Included

Affordable IVF will provide the following services for up to three egg retrievals:

• Facility and surgeon fees for egg retrieval and embryo transfer
• Ovulation induction monitoring including hormonal testing, ultrasound monitoring, and physician fees directly attributable to ovulation induction
• Anesthetic supplies and drugs
• All medical and surgical supplies required for egg retrieval and embryo transfer
• IVF laboratory services including identification of eggs, semen processing, fertilization of eggs (including ICSI), blastocyst culture, assisted hatching, embryo freezing, and frozen embryo storage fees while in active treatment (not all of these services will be needed by every couple)
• If pregnancy is not achieved in the fresh cycle, then this program will include subsequent thaw, culture, and transfer fees. Physician fees for transfer will also be included.
• Verification of pregnancy up to and including ultrasound verification of fetal number, position, and heart beat (services at outside facilities will not be covered).

What is NOT Included

Under this contract, Affordable IVF will provide all medical services for up to three egg retrievals with the following exclusions, which shall be paid by the couple:

• Medication cost except medication used for anesthesia
• All laboratory studies, evaluations, and surgery required to prepare the female partner for the start of the IVF process
• All care by physicians and other medical services not provided by Affordable IVF
• Professional fee for anesthesia for ovum (egg) retrieval
• Embryo and sperm storage fees once pregnancy is achieved
• Any complications related to the IVF process including, but not limited to treatment of hyperstimulation syndrome, pregnancy complications, miscarriage, or ectopic pregnancy.

Special Situations

Rarely, a woman will not respond to medications as predicted and will not meet the criteria for retrieval. If this happens in two stimulations, then the contract will be considered fulfilled. In this case, the deductible will be the actual cost of services provided, or the stated deductible, whichever is less. If medical conditions develop or are first discovered after acceptance into the program, we reserve the right to terminate the contract. • Female produces poor quality embryos with investigation revealing that her husband has a chromosome abnormality that makes success unpredictable
• Female develops medical condition making pregnancy or stimulation unwise
• Couple who displays antisocial or other behavioral problems that are not compatible with our continued treatment

Withdrawal from the Program

The Couple may withdraw from the program at any time. In this case, we will charge the Couple for services rendered only. These services will be charged at our usual rates, with the total not to exceed the original contract fee.
