Multi Cycle IVF Program

Pregnancy rates are now very predictable for most couples proceeding into IVF. However, there are situations were the potential for pregnancy is not predictable. For these couples we cannot provide the option of Refund IVF. However, we have another option to help limit and predict the total cost of the treatment. This is through the Multi Cycle IVF Program.

In this program you pay a set fee of $17,100. There is no refund in this program. You may have up to 3 IVF egg retrievals.

What is Included

Affordable IVF (AIVF) will provide the following services:

1. Ovulation induction monitoring including hormonal testing, ultrasound monitoring, and physician fees directly attributable to ovulation induction
2. Facility and surgeon fees for the egg retrieval and embryo transfer
3. Anesthetic supplies and drugs.
4. All medical and surgical supplies required for egg retrieval, and embryo transfer.
5. IVF laboratory services including identification of egg, semen processing, fertilization of the eggs (including ICSI as needed), blastocyst culture, assisted hatching, embryo freezing, frozen embryo storage fees while in active treatment. (not all of these services will be needed by every couple)
6. If pregnancy is not achieved in the fresh cycle then this program will include subsequent thaw, culture, and transfer fees. Physician fees for cycle management and transfer will be included.
7. Verification of pregnancy up to and including ultrasound verification of fetal number, position, and heart beat (services at outside facilities will not be covered).

What is not Included

Under this contract, AIVF will provide all medical services for up to three IVF cycles with the following exclusions which shall be paid by the couple:

1. Medication cost except medication used for anesthesia.
2. All laboratory studies, evaluations, and surgery required to prepare the couple for the start of the IVF process.
3. All care by physicians and other medical services not provided by AIVF (for example urology services, and ultrasound and laboratory testing not done in an AIVF facility).
4. Professional fee for anesthesia for ovum (egg) retrieval.
5. Processing of testicular biopsy specimens to recover sperm.
6. Embryo and sperm storage fee once pregnancy is achieved.
7. Any complications related to the IVF process including, but not limited to treatment of hyperstimulation syndrome, pregnancy complications, or ectopic pregnancy.

The contract will be considered fully performed by AIVF once delivery of a live born baby has occurred. Otherwise, this contract will be complete once three ovum retrievals have been done and all embryos transferred.

Withdrawal from the Program

This program was designed primarily for couples who’s potential for success is not predictable or is predictably poor. Therefore, withdrawal from the program will be permitted for couples who do not do well and do not want to continue. In all cases, you will be charged for services rendered and the remainder will be refunded.
