Tubal Reversal - Why Choose Us?
Tubal reversal by microsurgery. This had a major roll in why I chose Reproductive Endocrinology and Infertility and spent an extra 2 years training in the surgical and medical aspects of the specialty. When I am not doing reversals I do the ICSI (intracytoplasmic sperm injection) in my IVF lab. As far as I am aware, I am the only physician doing ICSI. This is the ultimate microsurgery. This is surgery on a single cell.
The current microsurgical techniques for tubal anastomosis/tubal reversal were developed in the late '70s. Originally this was done by interested gynecologists who went to their neurosurgery departments and learned microsurgery. By using the microscopic techniques and using microsutures this doubled the expected delivery rates. Most of these surgeons have now reached retirement age. Around this same time the subspecialty of reproductive endocrinology and infertility was developing. There was no IVF, so the training was highly surgical. We became the go to surgeons for complicated gynecologic surgery. We were the first specialists in micro-surgery, laparoscopic surgery, and hysteroscopic surgery. Because of this many of us were the first surgeons doing and teaching laparoscopic hysterectomy, laparoscopic pelvic reconstruction, and endometrial ablation. During the last 15 to 20 years, infertility training programs have not emphasized surgery. Many of the “new” specialists have never done a sterilization reversal. This is why women looking for tubal reversal get such mixed messages. These new docs honestly don’t know how well the surgery works. You therefore have to look for the surgeons in their 50s and 60s who trained during the golden era of microsurgery.
